This web page contains a number of interactive PDF Forms, which may be completed and then printed to a hardcopy printer, for forwarding by land mail, or to a PDF Printer for forwarding by email. To take best advantage of being able to print to a PDF printer, it is recommended that Google Chrome be used as the browser. Internet Explorer may not permit you to print the form and the data to a single document, whereas Google Chrome will. When using Google Chrome as the browser, fully complete the form, right mouse click somewhere on the form and select the print option, select the printer (PDF or otherwise), select the printing preferences (portrait or landscape, depending on the format of the form) and print the form to hardcopy or file. The resulting PDF Document will not be interactive, but will contain all of the data that was entered. If you do not have a PDF Printer, currently installed, I would recommend CutePDF Writer. It is free to download from the internet and is a very useful utility. I have been using it for many years and have not experienced any problems.
